JRS INTEGRITY LAB SERVICES — TERMS OF SERVICE

These Terms of Service ("Terms") govern your access to and use of the website, client portal, and services offered by JRS Integrity Lab Services ("JRS," "we," "us," or "our"). By accessing our website, scheduling a service, or using our client portal, you agree to be bound by these Terms. If you do not agree, please do not use our services.

1. SERVICES PROVIDED

JRS Integrity Lab Services provides the following services ("Services"), which may be updated from time to time:

- Drug and alcohol testing (lab-based and rapid panel)

- Background checks

- Fingerprinting

- DNA testing

- Mobile collection services

- Employer testing bundles and related compliance support

Services are performed by JRS staff or authorized collection partners in accordance with applicable industry standards and, where applicable, DOT/federal workplace testing guidelines.

2. CLIENT PORTAL ACCESS

Clients and employer accounts may be granted access to a secure online client portal to view test results, invoices, account history, and related documents.

- You are responsible for maintaining the confidentiality of your login credentials.

- You agree not to share your portal access with unauthorized individuals.

- You are responsible for all activity that occurs under your account.

- Notify us immediately at 219-533-8064 if you suspect unauthorized access to your account.

We reserve the right to suspend or terminate portal access at our discretion, including for suspected misuse, non-payment, or violation of these Terms.

3. SCHEDULING, PAYMENT, AND CANCELLATIONS

- Service pricing is provided at the time of scheduling or as outlined in your employer service agreement.

- Payment is due at the time of service unless otherwise agreed in writing (e.g., employer account billing).

- Invoices are payable within 1 day of the invoice date unless otherwise specified.

- Cancellations or rescheduling requests should be made at least 24 hours in advance. Late cancellations or no-shows may be subject to a fee.

4. ACCURACY OF INFORMATION

You agree to provide accurate, current, and complete information when scheduling services, registering for the client portal, or submitting any forms. JRS is not responsible for delays, errors, or issues arising from inaccurate information provided by you.

5. TEST RESULTS AND REPORTING

- Results are provided to the client and/or authorized employer/third party as designated in your service agreement or applicable consent form.

- Turnaround times vary by test type and are estimates, not guarantees.

- Certain results may require confirmation testing before being finalized.

- JRS follows applicable chain-of-custody procedures for regulated testing (e.g., DOT-required tests).

6. LIMITATION OF LIABILITY

To the fullest extent permitted by law, JRS Integrity Lab Services shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of our Services or client portal, including but not limited to lost employment opportunities, business interruption, or reliance on test results by third parties. Our total liability for any claim shall not exceed the amount paid by you for the specific service giving rise to the claim.

7. THIRD-PARTY SERVICES AND REFERRALS

Where applicable, JRS may work with third-party laboratories, collection sites, or background check providers to fulfill Services. JRS is not responsible for the acts or omissions of independent third-party providers, though we take care to work with reputable partners.

8. INTELLECTUAL PROPERTY

All content on the JRS website and client portal — including logos, text, graphics, and branding — is the property of JRS Integrity Lab Services and may not be copied, reproduced, or used without written permission.

9. TERMINATION

We reserve the right to refuse service, terminate accounts, or restrict portal access at our sole discretion, including for violation of these Terms, non-payment, or misuse of the platform.

10. CHANGES TO THESE TERMS

We may update these Terms from time to time. Continued use of our website, portal, or Services after changes are posted constitutes acceptance of the revised Terms. The "Effective Date" above reflects the most recent update.

11. GOVERNING LAW

These Terms are governed by the laws of the State of Indiana, without regard to conflict of law principles. Any disputes arising from these Terms shall be resolved in the applicable courts of Indiana.
